There are two options for you to provide information which is helpful. Depending on the depth of information you feel is important, you may choose which option is best for your needs. Click on Option 1 or send an email as described in Option 2.
Option 1: Use the HIPPA Compliant Form that is confidential and secure. You create your own user name and password (please remember as this is only available to you).
Once you have completed this in-depth form and hit submit, it is sent directly to Dr. Frances.
Option 2: Send an email to Dr. Frances at dore@dorefrances.com. Please include the following information:
- Your first and last name
- Your email address
- Your full street address
- Your cell number, home number, work number
- Your child's first and last name and their age, as well as date of birth
- A 4 line description of what you are interested in learning about
- A 4 line description of your current situation
After we receive your form and payment, we will contact you for a phone appointment.
Cost of service is $200 which includes one hour phone consultation and provided list of programs (70+) that have met standard background checks and are focused on assisting adolescents and teens on a low tuition basis.
This is for families that are not able to secure the full professional services of an educational / therapeutic consultant and who will meet standard criteria when applying for financial assistance and scholarship monies from the programs listed.
Additional hours are available when needed and requested.
For most of these programs parents will usually have to qualify for by disclosing their financial situation.
After we have received your consultation fee, you will be notified by email to set up a convenient phone appointment.
